How much should custom orthotics cost?

I've been given a prescription for custom orthotics, and while I know they can be expensive, I'm trying to figure out what a reasonable price range is so I don't get overcharged. The cost seems to vary wildly depending on the provider (podiatrist vs. physical therapist vs. online).
For those who have custom-made orthotics, what was the total cost you paid, and did that price include the initial consultation, casting, and follow-up adjustments? I see companies like Bilt Labs and Upstep do them from home and are cheaper but are they just as good? I have flat feet and was told I need them for arch support. I walk around alot at work so I need something durable that will last.
